4. Tuition Form 

5. Medical Form (Must attach immunizations)

6. Food Program Form

7. Medication Form (For children who need diaper cream or special diet; alternative milk requires a doctor's signature)

8. Medical Care Plan (For children who have health condition requiring special treatment; ex: inhaler, epi-pen; requires a doctor's signature)

9. Camp Registration Form
